- Acts as the customer service Liaison between Diabetic Management team and patient/family.
- Identifies and prioritizes potential utilization or continuity of care issues based on telephonic patient/family interviews.
- Recognizes when care is delayed or compromised and communicates to DM team based on urgency.
- Works within scope of practice. Understands when to escalate issues to a higher level.
- Coordinates the scheduling of Pharmacist consultations and ensures members receive timely appointment reminders.
- Daily support to Disease Managers in areas of authorizations, mailings, opening/closing EZcap cases, preparing ICT lists and reviewing hospice reports.
- Processes Nextgen exception reports, corrects errors on Mismatch report and processes daily schedules.
- Track and reschedule missed patient appointments and enters lab orders as needed.
- Assists pharmacists with logging in supplies and updating inventory spreadsheets and monitors Sample refrigerator temperatures.
- Assists with special projects.
- Other duties as assigned.
- High school diploma or equivalent required. Associates degree preferred.
- Two years experience in a physician's office, hospital or medical environment, preferred.
- Medical Terminology required. Proficient in Word, Excel, and Access.
- Strong communication skills, verbal and written.
- Detailed oriented.
- Able to function under pressure and multi-task effectively.
- BLS required for those with patient contact
- At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
