Desert Oasis Healthcare is Hiring a Patient Liaison Near Palm Springs, CA
- Provide continuous monitoring of quality measure reports for patient denominator and numerator status.
- Identify opportunities for patient outreach and determine priority for outreach efforts in collaboration with the Clinical Performance and Outcomes team.
- Provide direct outreach to patients who are numerator negative on quality measure report cards.
- Coordinate scheduling of laboratory testing, radiology testing or other screening tests for quality measure compliance (such as colonoscopy)
- Monitor patient follow through with testing and provide follow up with patient to identify barriers to completing quality measure activities. Diligently work to resolve barriers so that 100% of all quality measures are completed.
- May involve patient home visits to facilitate closure of quality measure gaps.
- Other duties as assigned.
- High School diploma or equivalent required. Associates degree preferred.
- A minimum of 2 years experience in a healthcare or managed care setting with knowledge of systems and or projects. Healthcare experience as a Phlebotomist, Pharmacy Technician or Certified Medical Assistant with Primary Care/Physician office experience required
- Must know or have the ability to understand multiple software programs including, but not limited to MS Office (Word, Excel), Business Solutions, DAI Scheduling, Ascender, EZCAP, NextGen, etc.
- Excellent written and oral communication skills.
- For those employees making home visits, BLS is required.
- At the discretion of DOHC/FHC management, this position has the potential to be a full or hybrid telecommuting position.
