Educates community on Denton County’s Public Health’s programs, services, and eligibility.
Educates community on health issues focusing on prevention, access to care, and local resources.
Assesses the community’s needs and assists with the development, implementation, and evaluation of activities to address health disparities.
Serves as a liaison between the community, health professionals, and health and human service organizations specific to community health.
Develops and evaluates program promotional and educational materials, including continuous assessment of accessibility and inclusivity.
Facilitates community-wide collaborative efforts and events specific to community health improvement.
Tracks, monitors, and documents activities for required reporting.
Synthesizes data to provide grant metric reports, local research efforts, and monitoring of health trends.
Leads focus groups to determine accessibility of communication and education materials specific to community health topics.
Regular and punctual attendance is required.
Performs other related duties as required.
Bachelor’s Degree and a minimum one year of experience in a related field; or an equivalent combination of education and related experience required.
Community Health Worker certification is required after 6 months.
Bilingual preferred but not required.
