Delta Health is Hiring a Clinic Patient Navigator - MA Near Delta, CO

The Patient Navigator focuses on the patient’s needs and performs a variety of health education, clerical and medical support services within the clinic setting and in the community. A primary function of the navigator is to establish and maintain communication between patients, their families, physicians, and the health care system to help facilitate care coordination that patients require.

  • Responsible for new patient intake appointments, patient care plans, medication prior authorizations and appeals,
  • TCM phone calls-submit referral to needed specialties
  • Responsible for patient outreach for preventive or chronic care services. Target patients will be patients with Hypertension, Diabetes, Cardiac Disease, ER Follow-ups, and Hospital Discharge follow-ups and other patients who require additional assistance.
  • Will ensure patients get to their appointments by reminding them of the day and time 2-3 days before appointment, and assist with any visit coordination that supports improvement in hypertension care and diabetes care.
  • Liaison for the patients and physicians in the practice by assisting patients in accessing service within the system, facilitating communication with referring physicians, referring patients to appropriate community resources and providing follow-up “tickler” clinical orders and scheduling.
  • Must complete background checking on all controlled substances, PDMP verification and manage all home health patients to include completion of billing forms required for certification and re-certification.
  • Clerical duties include: composes and types routine correspondence, copy and file correspondence, routes incoming mail, orders and maintains supplies and arranges for equipment maintenance; maintains and organizes file system.

Additional Clinic Duties:

  • Interviews patients, measures vital signs, such as pulse rate, temperature, blood pressure, weight and height, and records information on patient’s charts.
  • Rooms patients, obtains clinical information such as medication list, patient concerns, questions and height and weight.
  • Maintains skill level required to care for routine patients.
  • Prepares patient for and assists with examinations.
  • Maintains records of vital statistics and other pertinent data of patient. Conducts specified testing or calls medical facility or department to schedule patients for tests.
  • Physical examination, family, social, and past medical history as well as document procedures, lab results, dictated radiographic impressions made by the supervising physician and any other information pertaining to the patient's encounter ...
  • Acts a scribe for the provider when requested. Perform clinical documentation in EHR system. Ensure that all clinical data, lab, or other test results, as well as the interpretation of the results by the physician, are recorded accurately in the medical record.
  • Document procedures, consults, and discussions.
  • Makes phone calls to patients regarding test results or medication changes.
  • Administers injections and medications, dresses wound and incisions, performs blood draws
  • Anticipates patient needs and responds to patient requests promptly.
  • Assists with scheduling of patient appointments, answering incoming calls, typing and filing correspondence, ordering and maintaining supplies, arranging for equipment maintenance, recording of incoming payments and distributing billing as required.
  • Assists with special assignments as necessary.

Chart Extractor/Population Health Records Additional Responsibilities:

  • Responsible for extracting old records from EHR systems and adding items to Athena Health
  • Responsible for pulling health information from QHN and entering the data either as a structured data or as a formal document
  • Responsible for entering health history and problem lists based off of documentation that is extracted.
  • Five (5) years previous experience in a provider clinic setting or additional education or equivalent (i.e., Bachelor’s) with progressive experience required.
  • Must be currently licensed or certified/registered to practice as a CNA/MA/LPN in the State of Colorado
  • Ability to establish cooperative working relationships with leaders, physicians, peers, and the public.
  • Demonstrated ability to maintain composure when confronted by difficult situations and to respond professionally.
  • Demonstrated ability to work as part of a team while functioning independently, maintaining confidentiality in all assignments, and showing initiative in identifying and solving problems as they occur.
  • Experience with various computer software and hardware including word-processing, knowledge of Microsoft Word, Excel, Outlook, and the internet.
  • Ability to problem-solve with other members of our team and to effect change toward improvement of services.
  • Possesses strong customer service skills to promote pleasant and effective interactions with all of our internal and external customers, patients, staff, and physicians.
  • Skill in analyzing information, problems, situations, practices, and procedures to recognize alternatives and provide solutions along with recognizing an emergency or high priority situation and taking appropriate action.
  • Knowledge of medical terminology, and EMR required
  • Fluency in English (speaks, read and write) is required; Fluency in Spanish (speaks, translate, read and write) is preferred.
  • BLS is required
