The Nurse Navigator is responsible for assisting Oncology patients by navigating them through the health care system by helping to provide holistic care. During this process, the Navigator will collaborate with other members of the healthcare team, by promoting lifelong learning to improve the care of patients with a past or current diagnosis of cancer. The Navigator will address and connect the patient and families to any financial, psycho-social, educational, spiritual, and referral needs. In order to accomplish this, the navigator will oversee and direct patient centered programs to maintain the patients’ health and well-being; there will be daily communication with the Oncology/Infusion Director.
4 days/week, 9 hours/day
