Job Description

Dedicated Care Health Services operates its Core Service Agency in two locations, in the NE quadrant and the SE quadrants of the District of Columbia. The agency is a rapidly growing organization that seeks creative, caring and results oriented individuals who understand the importance of advocacy and care delivery.

Job Description

BASIC FUNCTIONS:
The Team Leader oversees the organizational structure and daily functioning of a community support team. This position will manage staff members who may serve a total caseload of up to 250 consumers diagnosed with a serious and persistent mental disorder including in some cases the dually diagnosed. The Team Leader reports to the Clinical Manager and is responsible for administrative processes ensuring the delivery of care in accordance with agency standards and Chapter 34.

PRINCIPAL ACCOUNTABILITIES:
 Ensures that all consumers are seen within within the required time frames MHRS In conjunction with the team’s Clinical Manager.

 Ensures that team members are hired, trained and supervised around their responsibilities.
 Works collaboratively with the multidisciplinary team including nursing, therapist and prescribers to ensure that a well rounded service delivery model is being adhered to.
 Responsible for conducting performance improvement monitoring in collaboration with the Clinical Manager. 
 Writes performance improvement plans when and if needed to ensure that all care matrices are met.
 Provides services in-person and via telehealth after hours and on weekends, if requested and as needed.
 Ensures that all documentation is complete and timely for each consumer.
 Monitors productivity targets for CSW and coaches if needed.
 Manages a professional staff of eight (8).
 May perform other duties as assigned.

SPECIALIZED KNOWLEDGE AND SKILLS REQUIREMENTS:
 Ability to coordinate with a multi-disciplinary team within an outpatient mental health agency.
 Knowledge of sound ethical clinical practice and principles related to the care of mental health consumers.
 A superior communicator, who shares information readily and concisely, listens as well as gives advice, and respects the abilities of others.
 Excellent written and verbal communication skills.
 Excellent computer skills, to include operating Microsoft Office Suite programs.
 Knowledge and ability to navigate an Electronic Medical Record (EMR), including data entry.
 Ability to relate to adults diagnosed with serious and persistent mental illness, using knowledge of
concepts, principles, and theories related to mental health and mental illness.
 Ability to perform amicably in an environment serving the seriously and persistently mentally ill adult
population.
 Ability to respect and uphold client confidentiality. Ability to exhibit a high level of organizational skills and able to prioritize and multi-task without direct supervision

Qualifications

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:
 A degree, licensure or certification in closely related field. 
 At least two (2) years of experience working with low-income individuals who have mental health disabilities.
 Possess a minimum of one (1) year of management experience
 Able to coordinate with a multi-disciplinary team.
