Daybreak Independent Services is Hiring an Intake Specialist Near Bronx, NY

Job Title: Intake Specialist
Reports to: Director of Intake and Social Services
Location: Bronx, NY
Job Classification: Full-time
Shift: Monday-Friday & some weekends, 35-40 hours per week
FLSA: Non-exempt
Salary Range: $23 -$25 per hour
The Intake Specialist is responsible for helping individuals through our agency's enrollment process. Intake Specialist reach out to each referral and help them navigate through the OPWDD Front Door process. In this important position, Intake Specialist accept the great responsibility of being the point person for the individual that is seeking services and their advocate throughout the entire process and work to get them throughout the process as seamlessly and efficiently as possible with a focus on ensuring all individuals are treated with respect and dignity by following person first processes. This position reports to the Intake Director.
Education & Experience:
  • Associate Degree in social work preferred or one year in human services, special education, or related field.
  • Spanish Speaking is a plus
  • Excellent attention to detail
  • Ability to get along well with co-workers and many administrative and management levels.
  • Capable of working under pressure and deadlines
  • Responsive for supervision
  • Flexibility and a strong work ethic; willing to take on additional tasks and work beyond regular work hours when required
  • Proficiency in Microsoft Office Suite, including Word, Excel, PowerPoint, and Access
Summary of Primary Responsibilities and Essential Functions of Position:
  • Adopts implements and promotes agency values and philosophy. Accepts referrals, supports the family during the referral process, obtains family history (including all intake and medical documents), and assures that Medicaid and documentation of a current diagnosis are in or pending.
  • Identifies client needs during initial contact with client and family. Also, you need to obtain additional information through telephone and other contacts as appropriate.
  • Prepares and enters information into the data system. Review possible plan of action in the event of ineligibility for case management services.
  • Schedule tours, new trials, observe trials, and lead the trials when necessary. Obtain and review that trial questionnaires have been properly completed from the program and clinical team.
  • Participate in Agency events such as DBIS Open Houses, Intake Fairs, CCO presentations as scheduled and requested by the supervisor. This includes but not limited to calling parents to confirm RSVP, delegating responsibilities to the Programs/Depts, providing documentation for parents, ordering marketing items for parents, speaking to parents as the events , and anything else that may be required to be done for successful event
  • Address intake concerns of parents regarding all services provided by the Agency.
  • Monitors all information and referral activities of families awaiting the assignment of a care manager as necessary. Has to contact all families on the wait list monthly.
  • Advocates for clients to protect their rights and entitlements as recipients of mental health services and as persons eligible for benefits such as special education, Medicaid, etc. Provides direct Assistance to clients and families in accessing and securing resources.
  • Maintains a record of all intake information, including but not limited to (Intake referrals, phone calls, sign-in sheets for parent visits, school visits, etc.).
  • Maintain client and family confidentiality rights in compliance with State and Federal laws.
  • Prepares and submits reports summarizing activities upon request
  • Providing support and information to families of the individuals we serve.
  • Provides Assistance to parents in communicating with program staff, Medicaid Coordinators, DDRO, other clinics, etc.
  • Assist as part of the DBIS Conflict Resolution Team.
  • Follow up with families every two months to discuss the individual's progress.
  • Perform other duties when requested.
  • Prepares and submits reports summarizing activities upon request
