Davis Behavioral Health is Hiring a Case Manager - Adult Mental Health Near Layton, UT

Job Description

Davis Behavioral Health, Inc. is actively recruiting a full-time Case Manager to work on the Adult Mental Health Outpatient team. This position will be based out of the DBH Layton office; however, the majority of the work will be conducted in the community throughout Davis County.

As part of a multi-disciplinary team, the Adult Mental Health Outpatient Case Manager is a bachelor's level mental health professional who provides targeted case management, behavior management, skills development, and personal services to adults with serious mental illness and co-occurring substance use. The case manager assists clients in effectively utilizing available community resources. Case management can be provided in the home, office, and community settings. This position offers hope, guidance, and skills toward obtaining and maintaining thriving independence in community living.

Essential Job Functions

The Adult Mental Health Outpatient Case Manager must be able to complete the following essential job functions:

  • Complete DLAs (or other agency identified case management needs assessments) at the required frequency. that drive individualized goals and objectives.
  • Use information from the DLA to complete individualized service plans in support of overall mental health treatment goals.
  • Maintain agency standards for productivity and documentation.
  • Engage with clients' treatment team to support clients and coordinate treatment objectives.
  • Maintain active caseload.
  • Coordinate and advocate for care that ensures basic needs are met. This may require coordination and advocacy with multiple local and State agencies.
  • Work with clients and their families or other support systems to ensure basic needs are met.
  • Be knowledgeable about the crisis system and know when to activate it.
  • Able to balance independent work while appropriately consulting with supervisor and taking directions when given.
  • Other duties as assigned.

Strong preference will be given to candidates who graduated from a college or university with a bachelor's degree in social work (BSW) AND active licensure as a Social Service Worker (SSW) in good standing in the State of Utah.

Employment Classification & Schedule

This position is classified as full-time, non-exempt.

The proposed schedule for this position is Monday through Friday from 8:00 am to 4:00 pm.

Location

The DBH Layton Campus is located at 2250 North Robins Dr., Layton, UT 84041.

Compensation

The hourly rate for this position is outlined below:

BSW Degree SSW License

  • $23.00 per hour base wage 1% per year of licensure
    • BSW degree must be recognized by DOPL to allow for licensure without completion of additional coursework

Non-BSW Degree

  • $19.00 per hour base wage 1% per year of direct social work experience, up to 10 years.
  • $1.00 per hour added to base rate if licensed as an SSW in the State of Utah.

No Degree TCM Certification

  • $17.75 per hour 1% per year of direct social work experience, up to 10 years.

Receiving Services at DBH

It is highly preferred that individuals who are interested in obtaining employment at DBH, their spouse, or one of their dependents not be actively receiving services at DBH except for medication management services to eliminate/avoid possible conflicts or uncomfortable boundaries.

Introductory Period

All new employees at DBH have a 6-month introductory period to assess whether they can perform the essential functions of the job at the expected level, within the specified timeframes, and are a good fit for the job.

Required Qualifications

Education/Licensure/Certification

Graduation from a college or university with a bachelor's degree in social work (BSW) AND active licensure as a Social Service Worker (SSW) in good standing in the State of Utah.

AND

The Adult Mental Health Outpatient Case Manager must complete and maintain certification in First Aid and CPR.

Underfill Options

Position may be under-filled by an individual who has graduated from a college or university with a bachelor's degree in another social science field (i.e., psychology, criminal justice, sociology, etc.) AND has active licensure as a Social Service Worker (SSW) in good standing in the State of Utah or can meet criteria to obtain licensure within 12 months of hire.

OR

Position may also be underfilled with someone who has the requisite experience in working with seriously mentally ill adults and is able to obtain and maintain certification as a targeted case manager.

Experience Required

Demonstrated experience in the following areas is essential:

  • Ability to work diligently and maintain company standards without prompting.
  • Ability to follow written and oral instructions.
  • Ability to develop and maintain effective working relationships with colleagues and clients.
  • Ability to communicate effectively orally and in writing.
  • Ability to motivate and engage clients.
  • General understanding of mental illness and substance use disorders.
  • Ability to maintain client confidentiality.

The ideal candidate will also have a working knowledge of mental health and substance use disorder services, community resources, experience working with clients that have severe mental illness, is hardworking, and can work independently as well as in a team environment.

Skills/Abilities/Qualifications

The Adult Mental Health Outpatient Case Manager must possess the following skills, abilities, and qualifications to successfully perform the essential functions of the job:

  • Ability to meet or exceed productivity standard set by DBH.
  • Proficiently utilize required systems and/or equipment related to performing essential functions of the job.
  • Ability to independently manage schedule and assigned caseload, including scheduling appointments with clients, attending required meetings, coordination of care, supervision, and other work activities related to the completion of essential job functions.
  • Must have reliable transportation and a valid driver's license.
    • A clean driving record is also required to utilize the DBH vehicle fleet.
  • Ability to assist clients in developing a range of social supports in the community.
  • Ability to advocate for client rights.
  • Ability to assist clients in monitoring their medications.
  • Skill in the application of problem-solving and multitasking.

All employees at DBH must successfully pass a pre-employment criminal background check as well as a pre-employment drug screening. Since Davis Behavioral Health works with Federal grants the company and its employees must comply with the Federal drug laws, including no use of medical marijuana.

Preference will also be given to individuals who:

  • Have good organizational and practice management skills.
  • Can work independently and effectively.
  • Be flexible in days and hours available for scheduled work to best meet the need of clients and DBH.

Physical Demands

Tasks require a variety of physical activities, not generally involving muscular strain, but do require frequent walking, standing, stooping, sitting, reaching, talking, hearing, and seeing. Common eye, hand, and finger dexterity are required for many tasks. Mental application utilizes memory for details, verbal instructions, emotional stability, and discriminating thinking. Periodic interaction with youth or adults in crisis requiring participation in de-escalation processes may occur causing stress or threat of physical injury.

DBH Criminal Background Check & Drug Screening Disclaimer

Davis Behavioral Health is a drug-free workplace and requires all employees to successfully pass a pre-employment drug screening. DBH will not hire individuals currently using illegal drugs or abusing alcohol; therefore, making employment at DBH conditional upon successfully passing pre-employment drug screening by providing a valid sample.
