The Registered Nurse Case Manager assumes full responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered. Qualifications:

§ Current Registered Nurse (RN) licensure (in good standing) in the state of Arizona

§ Works well within a team environment to collaborate daily with interdisciplinary team.

§ Can work independently and on your own on a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.

§ Time management skills to manage visits, documentation, and meetings.

§ Ability to learn electronic medical records system.

§ Must have reliable transportation.

§ Continuously evaluates personal and professional performance and makes necessary changes to increase productivity and quality of care delivered.

§ Adheres to and supports the policies/procedures/goals/objectives of hospice to assure quality patient care.

§ Strives for independence and sound critical decision-making regarding the delivery of patient care in an effective manner without compromising quality.

Pay: $85,000.00 per year
