Primary Job Functions & Responsibilities:
- Promote the level of clinical expertise required of staff to provide safe, high quality nursing care.
- Performs appropriate documentation to maintain the standards set by the Agency - complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
- Evaluates/assists in evaluation of appropriateness of plan of care and revise on an ongoing basis with communication to all other disciplines.
- Plans, teaches, supervises and counsels family and/or patient's caregiver to promote improvement/recover, symptom/complication management, expected disease progression, social/emotional care and community resources.
- Prioritizes nursing visits based on the patient’s health status and anticipated needs.
Education & Experience Requirements:
- Graduate of an accredited School of Nursing.
- Two (2) years nursing experience required.
- One (1) year Home Health experience preferred.
- Wellsky/Kinnser experience preferred.
Licensure and/or Certification:
- Current RN license in good standing to practice in the state of Arizona
- Current CPR Certification
- Current Arizona Level One Fingerprint Clearance Card
- Valid driver’s license with maintenance of safe driving record and a reliable automobile that is insured in accordance with state requirements.
- Wound Care Certified, preferred
- Infusion Certified, preferred
