We are currently seeking qualified applicants for our Full-Time Admissions Screener vacancy.

The Admissions Screener is responsible for oversight of all aspects of the assessment process of a patient’s admission into a sub-acute rehabilitation or long-term care in a skilled nursing facility, including conducting preliminary screening of admission candidates (patients), visiting patients, educating interested parties about the services offered, and deciding whether they are suitable for admission to Daughters of Sarah. The Admissions Screener also performs a broad range of admissions functions including data entry, phone calls, processing admission applications, and obtaining authorizations.

The Admissions Screener is responsible for establishing and maintaining open, effective communication between Daughters of Sarah and referring publics, consumers, physicians, and third party payers; providing maximum flexibility in coordinating the payer’s needs with patient outcomes; communicating effectively with the treatment team, and influencing the treatment approach when needed; and achieving census targets by working in collaboration with facility leadership. Will travel to area hospitals and assisted living to screen medical record for potential residents .

Will train the right candidate but it is preferred that you have experience in healthcare.

Requirements:

• Associate's Degree and or two years or more of healthcare experience
• Current LPN or RN License highly preferred
• Prior experience working in hospital, acute/sub-acute care and/or long term care highly preferred.
• Must have excellent verbal and written communication skills including proficiency with computers
