We build communities from the inside out through community engagement and action, workforce development, outreach, community healthcare services, and alternative housing.

Accountable to Housing Program Manager

Compensation/Benefits:

• Full-time, non-exempt position: $26 hourly
• 100% employer-paid medical, vision, and dental premiums.
• Eleven paid holidays, vacation, and sick time.

Duties/Responsibilities:

• Assist neighbors in locating appropriate and affordable housing.
• Advise and direct neighbors in the development and implementation of an action plan for securing housing.
• Maintain good working knowledge of available housing in the community.
• Initiate and maintain communication with Case Managers in supporting neighbors' success in sustaining housing.
• Serve as a liaison with landlords to achieve stable housing for neighbors.
• Ensure prompt case notes, documentation, and data quality within HMIS (Homeless Management Information Systems) and other organizational data and case management systems.
• Assist in tracking guest outcomes and service trends in order to promote on-going evaluation and improvement of services.
• Advocate for clients in unsafe or neglected properties.
• Conduct housing workshops.
• Provide training and support around housing options to Case Managers.
• Represent Cultivate Initiatives to partners, Joint Office of Homeless Services, and the broader community.
• Attend all assigned trainings and meetings, unless otherwise approved.
• May participate in special projects and activities.
• May perform other duties as requested by Housing Program Manager and Housing & Sheltering Director

Requirements:

• Must be able to provide proof of COVID-19 vaccination.
• Must be able to provide proof of valid Driver’s License.

Physical Requirements:

• Ability to sit, stand or walk for prolonged periods, and ability to stoop and kneel occasionally
• Ability to speak and hear to use telephone
• Ability to effectively use computer, lift paper and files
• Ability to push, pull reach for and lift articles up to 10 pounds
• Ability to safely operate a motor vehicle
