GENERAL DESCRIPTION OF POSITION:
Under the direct supervision of the Community Services Director, the Community Services Deputy Director or Program Coordinator, the CRS is responsible for case management of families and individuals eligible for the assigned Program, other Community Services Department programs, and services, and for assisting in the development of programs and services, as needed for low-income families and individuals. Responsible for outreach, eligibility determination, records management, referrals, follow-up services, identifying and securing community resources for participants, maintaining partnerships, and representing the agency at collaborative and partner meetings. Performs applicant intake and eligibility determination activities, identifying and assessing the eligibility of families and individuals as specified in program guidelines. Explains regulations, policies, or procedures to applicants, participants, staff, and/or partner agencies as needed. Interviews families and individuals to identify strengths, goals, needs, and critical issues/barriers by conducting a needs/assets-based assessment as well as a barrier to stability/self-sufficiency assessment.
Conducts follow-up on participants who have completed or exited training or programs or received services on a regular basis to measure outcomes and receive feedback. Desired awareness of the problems in the community and knowledge of the target area/population to be served.
QUALIFICATIONS:
BS or BA degree in one of the social sciences or related fields or at least two years of college and four years experience in a similar position. Any work and/or life experiences that contribute to maturity, sensitivity to others, and motivation to work in a Community Action Agency are helpful. Must have a valid driver’s license, good driving record, and reliable vehicle and motor vehicle insurance coverage.
