RESPONSIBILITIES OF A MDS Coordinator:
- Conduct comprehensive assessments of patients using standardized tools and protocols
- Collaborate with interdisciplinary team members to develop individualized care plans
- Ensure accurate and timely completion of Minimum Data Set (MDS) assessments
- Monitor and update patient records to reflect changes in condition or treatment plans
- Coordinate with healthcare providers to obtain necessary documentation for accurate coding and billing
- Review medical records for completeness, accuracy, and compliance with regulatory standards
- Provide education and training to staff on documentation requirements and best practices
- Participate in quality improvement initiatives to enhance patient outcomes
QUALIFICATIONS OF A LPN:
Must have an active RN/ LPN License in the state of Ohio.
