Requirements

Education

• MD or DO
• Successful completion of an approved residency training program in Family Medicine or Emergency Medicine.
• Board Certification or board eligible for certification in Family Medicine or Emergency Medicine: Required

Experience

• Must be able to demonstrate that he or she is currently engaged in an active medical practice in Family medicine or Emergency medicine and has worked at least 600 hours in an Emergency Room in the last 2 years; or that he or she has recently competed a residency in family medicine or emergency medicine.
• Those desiring to hold emergency medicine privileges must also be able to demonstrate their practice includes the provision of care to family medicine patients; and those recently completing an approved residency program must be able to demonstrate the appropriate training was included in the residency program.

Computer Skills

• Previous experience with EPIC helpful but not required.

Certifications & Licenses

• Must hold a valid, unrestricted , current medical license to practice medicine issued by the State of Wisconsin, a narcotics license and be credentialed through the Medical Staff of the hospital.
• Must have Advanced Cardiac Life Support (ACLS), Advanced Trauma Life Support (ATLS) and Pediatric Advanced Life Support (PALS) certifications.
