Crossing Rivers Health is seeking a Family Nurse Practitioner/Certified Nurse Midwife to join our newly constructed Primary Care Clinic in Prairie du Chien, Wisconsin. We are a Critical Access Hospital and Clinic nestled in the heart of the Midwest in a thriving historic river town surrounded by picturesque bluffs. Crossing Rivers Health is a comprehensive community health center providing more than 100 services to the people who live throughout Southwest Wisconsin and Northeast Iowa.

We are seeking a seeking a FNP/CNM to join our extraordinary team who has a strong community commitment to health and wellness. In this role you will be responsible for providing care to patients while working both independently and collaboratively with other members of the healthcare team.

Requirements

Education: 

• Master's Degree Required, Field of Study: Nursing 
• Doctorate Preferred, Field of Study: Nursing
• Completion of an educational program in nurse-midwifery approved by the American College of Nurse-Midwives

License & Certifications: 

• Licensed as a Registered Nurse in the State of Wisconsin
• Licensed as an Advanced Practice Nurse Prescriber (APNP) in the State of Wisconsin
• Certification or able to obtain certification as a Nurse-Midwife in the State of Wisconsin
• Certification or able to obtain certification from the American Midwifery Certification Board
• Current DEA number
• Must be Board Certified

Experience: 

• Minimum of 1 year experience in a Family Practice setting as well as experience with both inpatient and outpatient OB services, prenatal care and women's health issues - Preferred 

Other Requirements: 

• Licensing must be obtained prior to first day of employment
• The Nurse Practitioner must have a collaborative agreement with a physician(s), who is a member of the Crossing Rivers Health Medical Staff.
