Description

Position Summary

The Behavioral Health Case Manager is responsible for providing case management services, completing comprehensive intake assessments and facilitating individual and group counseling for patients served through the Community Care Campus and the Macon County Jail. This individual consults with medical and clinical staff on substance use and behavioral health issues of patients.

Position Status

This is a full-time, exempt position. The Behavioral Health Social Worker reports to the Residential and Outpatient Supervisor/Director of Behavioral Health Services.

Qualifications

Education and Experience

  • Bachelor’s Degree in social work, psychology or related field
  • Certified Alcohol and Drug Counselor (CADC) is a plus
  • Successful work history providing direct patient care in a social services or healthcare setting is a plus

  • Ability to maintain confidentiality
  • Ability to relate to persons of various economic levels and cultures
  • Ability to remain calm under stressful conditions, accepting personal limitations
  • Ability to speak and write fluently in English; Spanish desirable
  • Ability to work independently and interdependently within a clinic setting utilizing various accepted therapeutic modalities, adherence to a strong code of personal and professional ethics
  • Knowledge of and access to community resources
  • Proficiency in data entry, interpersonal, organizational and problem solving skills
  • Proficiency in working in an Electronic Medical Record (EMR)
  • Willingness to pursue continuing education opportunities

Physical Requirements

  • Ability to work in conditions that include exposure to blood, bodily fluids and tissues, contagious diseases and/or other potentially hazardous materials
  • Moderate physical exertion including frequent walking, standing, bending, reaching and carrying moderately heavy loads (up to 25 lbs.)

Essential Duties & Responsibilities

  • Manages the care of patients in collaboration with other health professionals and members of the healthcare team in order to make decisions about treatment and attain health goals
  • Identify community resources and make appropriate referrals to maintain continuity of care
  • Facilitates/Co-Facilitates group therapy for patients with substance use or dependency
  • Completes SUPR approved, comprehensive intake assessments with patients
  • Collaborates with and advocates for patients and all involved Behavioral Health entities
  • Collaborates with other community care providers in referral linkage
  • Receives Behavioral Health therapy referrals, provides counseling, maintains and records all pertinent patient information in a secure and accurate manner
  • Vigilant to safety concerns and takes appropriate steps to ensure both patient and staff safety
  • Performs other duties as required
