Job description
A paraprofessional person who is specific call trained, competent and performs assigned functions of personal care to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN).
Policy: Major Duties/Responsibilities
Personal hygiene & care
· Bathing: bed, tub, shower, complete supervision of the client bathing activities.
· Oral hygiene, including denture care
· Hair care
· Shaving
· Skin care
· Nail and foot care (cleaning only, no cutting)
· Dressing and grooming Mobility
· Turning and positioning
· Assisted transfers and ambulation; with and without assistive devices
· Encourage coughing & deep breathing
· Measure intake and output (I&O)
· Assist with use of bedpan, bedside commode, toileting activity
· Incontinent care (bladder/bowel)
· Catheter care, limited to cleansing and positioning of external parts of drainage system and emptying drainage system Nutrition
· Encouraging and facilitating adequate nutritional and fluid intake
· Recording nutritional and fluid intake as requested
· Bed making; occupied and unoccupied including linen change
· Trash removal
· Safety
· Identify and report hazards to immediate supervisor
· Eliminate safety hazards for clients
· Reality orientation and sensory stimulation
· Listen and converse
MINIMUM QUALIFICATIONS
1. Must be high school graduate or GED
2. Must have a STNA certification in good standing
3. Current CPR/First Aid certification
