Creekside Post-Acute is a 130-bed skilled nursing facility that specializes in short-term rehabilitation and skilled nursing. We have been serving San Jose, California for over 50 years! We are looking for a committed, caring, and reliable Social Service Assistant to join our Skilled Nursing Facility!

Reports directly to: Director of Social Services and/or Social Service Lead

PURPOSE OF YOUR JOB:

Your responsibilities will include assisting licensed social workers, providing psychological, rehabilitation, and social support to families, and guiding individuals to find suitable benefits and social services. To be a successful social worker assistant, you should have excellent communication, interpersonal, and problem-solving skills. Ultimately, a top-notch social worker assistant is compassionate and empathetic and has high emotional intelligence.

  • Keep accurate, updated, and organized records after every client visit to ensure that there are no undocumented breaks between treatments or visits.
  • Answer questions about benefits that are available to individuals, or refer them to the correct organization to get help.
  • Discuss benefits like child care, food stamps, housekeeping, sanitation, and money management to determine needs versus wants.
  • Review and submit reports regarding any problems with a customer or a supervisor.
  • Get together weekly with supervisor to brainstorm and come up with ways to improve programs for individual families within the community, and to determine better ways to serve citizens.
  • Interview family members or individuals to obtain a complete family history to more closely determine which services are appropriate and needed.
  • Oversee group activities on a daily basis and run meetings when a social worker can’t be present.
  • Meet with youth groups and at-risk groups in the area to teach them about the consequences associated with drugs, violence, and delinquency to encourage them to make good choices.
  • Social Worker Assistant Requirements:
  • An associate's or bachelor's degree in social work, social sciences, human services, or related will be preferred.

EDUCATION:

  • A high school diploma or GED is REQUIRED
  • Associate’s degree in a related field is PREFERRED
  • Possession of certification in social services for long-term care is desirable.

EXPERIENCE/QUALIFICATION:

  • Experience in the healthcare field is PREFERRED
  • Extensive experience in Customer Service is REQUIRED
  • Experience in Data Entry Software, Assessment Tools, and Tech Savvy is PREFERRED
  • Interest in working in Skilled Nursing Facility and Social Service Department
  • Flexible hours and ability to work nights and weekends; Willing to work a flexible schedule.
  • Experience in social work may be advantageous.

KNOWLEDGE/SKILLS/ABILITIES:

  • Experience in customer service and be comfortable communicating with clients, their families, and medical teams.
  • Knowledge about any of the following areas: marriage, speech, sociology, personal health, human biology, or casework methods
  • Good computer and organizational skills.
  • High emotional intelligence.
  • Strongly compassionate and empathetic.
  • Excellent communication, interpersonal, and problem-solving skills.

PHYSICAL REQUIREMENTS:

  • Physical requirements of bending, squatting, twisting, stair climbing, walking, sitting, standing, lifting, and carrying items up to 35 lbs.
  • Ability to use equipment and techniques to safely assist patients to transfer, move in and out of bed, and ambulate.
