Creekside Post-Acute has been serving San Jose, California for over 50 years. It is a 130-bed that specializes in short-term rehabilitation and skilled nursing. We are seeking a dedicated and experienced registered dietitian to join our team in a skilled nursing facility!

RESPONSIBILITIES:

• Conduct nutrition assessments and develop individualized care plans for residents
• Provide ongoing monitoring and evaluation of residents' nutrition status
• Implement interventions to address residents' nutrition needs, including dietary modifications and nutritional supplements
• Collaborate with healthcare team to ensure comprehensive care plans are developed and implemented
• Educate residents and their families on proper nutrition and dietary guidelines
• Monitor and document food and fluid intake, and address any issues or concerns
• Collaborate with culinary staff to create and modify menus to meet residents' nutritional needs

REQUIREMENTS:

• Bachelor's degree in dietetics or nutrition
• Current registration as a dietitian with the [appropriate regulatory body] in the state of California
• Is in good standing with all regulatory agencies and licensing boards is required
• Interest in working with the geriatric population
• Strong knowledge of geriatric nutrition and the dietary needs of older adults
• Familiarity with state and federal regulations related to nutrition and dietary services in long-term care facilities
• Excellent communication and interpersonal skills
• Ability to work effectively within a multidisciplinary team
• Strong organizational and time management skills

EXPERIENCE:

• New passer/graduates are welcome
• With Skilled Nursing or Long-Term Care experience is preferred
• Experienced Registered Dietitian (RD)
