Creative Support is Hiring a Supervising Case Manager/Lead Personal Agent Near Medford, OR

The Lead Personal Agent (Lead PA) provides direct brokerage services to individuals with developmental disabilities following the rules and guidelines set forth by the Oregon Administrative Rules for Support Services and Creative Supports, Inc. Policies and Procedures. This is an exempt position. The position schedule is typically 8:00 a.m. - 5:00 p.m. Monday through Thursday with a 30-minute meal break. Friday will be 8:00 a.m. – 2:30 p.m. Fridays with a 30-minute meal break. The position is responsible for supervision of Personal Agents, mentoring and training. Works closely with the Executive Director to support a positive team environment and to assure the highest level of quality in the services provided to the customers of Creative Supports, Inc. This is an at-will position.

During the time of COVID, or other similar public health emergencies, the employee will work in a combination of in office and work from home as the Governor’s orders mandate and as quality services to customers is met. The work from home during these times is granted at the sole discretion of the Executive Director and may be rescinded at any time within the Executive Director’s sole discretion. Work schedule will remain as assigned. Must comply with Governor’s safe practices mandates. Must continue to exercise safe work practices.

Minimum Qualifications:

· Bachelor’s Degree in Business Administration, Psychology, Social Work or related field, as well as appropriate management experience, and experience in programs serving people with developmental disabilities or a combination of five years’ experience and education to meet the above criteria and the PA qualifications in the OAR

· Meet all personnel criteria outlined in the Policies of Creative Supports, Inc.

· Experience supervising staff.

· Experience training staff.

· Demonstrates effective coaching skills.

· Ability to maintain effective and professional working relationships with Executive Director, Staff, Board of Director’s and community partners.

· Demonstrates understanding of Medicaid and OAR compliance.

· Demonstrates understanding of and compliance with operating policies and procedures.

· Effective time management skills including managing deadlines.

· Satisfactory Criminal History check meeting SPD requirements.

· Valid Oregon driver’s license, satisfactory driving record, auto liability insurance and reliable transportation.

· Demonstrates ability to effectively communicate orally and in writing.

· Demonstrates ability to effectively work as a positive team member.

· Proficient with Microsoft Office Modules; Word, Excel and Access.

· Willing to travel as needed.

MAJOR DUTIES AND RESPONSIBILITIES:

· Provide supervision to Personal Agents (PA).

· Assure that appropriate rules, policies and practices are followed by PA.

· Provide corrective action where necessary.

· Conduct annual personnel evaluations for PA’s that are supervised by this position.

· Monitor team members leave.

· Conduct regular team meetings.

· Promote positive and supportive interaction among team members.

· Promote safe productive work environment.

PA Support:

· Supports PA’s/office staff to perform their jobs to the highest level of quality.

· Mentors PA’s/ office staff to help them build their skills and work as a team.

· Regularly directs the work of two or more staff.

· Provides critical recommendations on the hiring, discipline, firing, advancement, and promotion of supervised staff.

· Is available to consult on conflict issue as needed to support PA’s.

· Provides training/mentoring plan to PA's at time of hire and ongoing throughout their employment as needed or requested by ED.

· Assures Executive Director is informed of all performance concerns and complaints.

· Promote training for individual needs.

· Utilize ISP reviews to determine coaching needs.

· Act as communication liaison between ED and PA’s.

QA/QI/Compliance:

· Review teams ISP’s and give feedback for compliance.

· Assist in development and implementation of Practice Protocols.

Resource and Capacity Building:

·Participates in agency plan for resource development and capacity building.

· Positively represents the brokerage and the brokerage interests at community meetings.

· Positively represents the brokerage for presentations as requested.

· Attend statewide meetings as requested.

Assessment Oversite:

· Provide oversite and support to assessment team.

· Attend training as required on assessment processes.

· Assist to develop assessment team standards.

· Maintain assessment credentials through appropriate training.

Special Duties Lead ONA, CAM and Others as Assigned:

· Lead CAM staff in related duties.

· Lead CAM team in regular meetings.

· Compile reports as needed or required.

· Maintain ONA assessor standard.

· Assure ONA team maintains the highest level of training and implementation standards for ONA and DRC.

Professional Conduct:

· Respectful of the needs of the work schedule. This includes using work time productively, complying with assigned work schedule and using leave time prudently.

· Assure ED or designated supervisor is informed of schedule.

· Assure ED or designated supervisor is updated on work duties and special assignments.

· Exhibits professional appearance and professional conduct at all times. This includes maintaining positive communication that is supportive of the organization and all of the organizations staff and board members.

Miscellaneous Responsibilities:

· Attend all meetings and training opportunities as needed or requested.

· Provide back up support in the absence of the Executive Director

· All other duties as assigned.
