REHABILITATION ASSISTANT
DEPARTMENT:REHABILITATION
RESPONSIBILITY: DIRECTOR OF REHABILITATION
JOB SUMMARY: The purpose of this position is to carry out the restorative procedures as planned by Restorative Nurse on specific residents; assist the resident in attaining and maintaining the highest level of function possible in the performance of restorative nursing procedures in accordance with the resident's needs, goals, and abilities.
QUALIFICATIONS:
1. Must hold a current Certified Nurse's Assistant license in the State of New Jersey.
2. Completion of a Rehabilitation Nursing course approved by the State of New Jersey is preferred.
3. Must be able to read, write, and speak English in an understandable manner.
JOB REQUIREMENTS:
1. Must be physically and mentally capable of performing routine, repetitive job duties.
2. Competency in rehabilitative and restorative care
3. Show willingness to provide good rehabilitative and restorative care.
4. Use skill and understanding in handling residents.
5. Be considerate, pleasant, and professional in the performance of his/her duties.
6. Must have compassion, tolerance, and understanding for the elderly.
7. The ability to read and write effectively and to communicate effectively with the residents and other staff.
MAIN DUTIES:
A. Support the facility's philosophy of care and strive to achieve its goals and objectives.
B. Carry out rehabilitation/restorative procedures as planned by the Restorative Nurse recommended by the physical/occupational/speech therapist such as the following:
a) ROM-AROM, AAROM, PROM
b) exercises self-ranging, wheelchair pushups, standing tolerance, etc.
c) ambulation
d) positioning bed, chair, etc.
e) transfer training
f) ADLs
g) application of assistive devices and special programs
C. Document all program delivery and resident self-performance during program on POC/EMR system.
D. Promote and encourage daily care of resident by implementing approved rehabilitation/restorative techniques and training programs.
E. Encourage maximum functional level of performance with all ADL’s carrying out programs developed by Restorative Nurse.
F. Participate in ongoing staff education, in-services, and on-the-job rehabilitation/restorative training programs as scheduled or as directed.
G. Manage the rehabilitative/restorative equipment such as special mattresses, wheelchairs, walkers, cushions, restraints, splints, etc.
H. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess equipment, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day.
I. Assure that established infection control and universal precaution practices are maintained when performing rehabilitation/restorative procedures.
J. Follow established safety precautions when performing tasks and when using equipment and supplies.
K. Assure that equipment is cleaned and prepared for the next shift.
L. Report all equipment malfunctions and breakdowns to the Director of Rehabilitation as soon as possible and keep the DOR informed of supply needs and equipment needing replacement. Report all hazardous conditions or equipment to the Director of Rehabilitation.
M. Assure that work schedules are followed.
N. Maintain the confidentiality of resident information and honor his/her personal and property rights.
O. Attend rehabilitation department meetings weekly, notifying Restorative Nurse of any changes in resident status, participation, or tolerance of programs.
P. Turn in Days Off and payroll special request forms timely.
Q. Follow established fire, disaster, safety, infection control, and evacuation policies and procedures.
R. Perform other duties as assigned by the Director of Rehabilitation.
