CPC Behavioral Healthcare is Hiring a Program Coordinator, Community Support Services (Full-Time, Eatontown) Near Eatontown, NJ

CPC Integrated Health is looking for a Program Coordinator that is a Licensed, Master's Level clinician whose goal is to establish and maintain a quality rehabilitation service program in compliance with NJ Division of Mental Health Services and Addiction Services (DMHAS) and The Joint Commission (TJC). The Program Coordinator works closely with other coordinators, supervisors, and staff to develop policies and procedures, monitor the effectiveness of the Community Support Services (CSS) program, and maintain a working relationship with other service providers to ensure that a continuum of care is conducive to meeting the needs of the seriously mentally ill (SMI) population. The Program Coordinator assures services are delivered in a recovery-oriented and client-centered manner. The Program Coordinator ensures that CSS and supported employment is integrated along with clinical services as part of a holistic treatment plan.

MINIMAL QUALIFICATIONS:

  • NJ License and Master's Degree in clinically related field such as social work or counseling
  • LSW or LAC Certification
  • Minimum of 2 years in a community behavioral health setting

PREFERRED QUALIFICATIONS:

  • Experience with evidence-based practices in psychiatric rehabilitation


RESPONSIBILITIES:

  • Complete assessments and service plans that are age appropriate, culturally sensitive and recovery focused. Ensure that service plans are individualized to a client's needs and are focused on linking clients to specific community services to meet those needs (housing, entitlements, treatment services etc.)
  • Ensure compliance with agency policies and procedures and regulatory agencies
  • Provide administrative and clinical supervision to the staff. Work cooperatively and in coordination with peers, subordinates, Executive Staff, and community organizations and institutions
  • Participate in hiring, ensure staff competencies, and oversee program orientation for staff.
  • Assure that assessments and services consider developmental stage and age of clients served. Assure treatment that includes an assessment of client and family's psycho-educational learning needs and provide appropriate services.
  • Participate in problem resolution with clients, family/significant others, and other service providers.
  • Approve delegated responsibilities related to purchasing, billing and physical plant needs related to the assigned programs.
  • Conduct regular administrative reviews of case conferences.
  • Manage and conduct client intake process and assignment of cases.
  • Conduct program analysis, assure Continual Quality Improvement (CQI), and provide periodic reports on the results of program activities.
  • Work on committees as assigned.
  • Work with Director of Training to identify staff training needs and participate in ongoing staff training.
  • Communicate effectively in speech and in writing.
  • Assist with the preparation and management of operating and/or capital budgets for the programs.
  • Assist in the development of policies and procedures, development of grant proposals, funding requests and service plans as requested.
  • Work with supervisors around program scheduling activities to ensure appropriate program coverage.
  • Work to ensure that individual productivity standards are met.
  • Assure staff compliance with monitoring agency requirements and procedures regarding documentation and billing are met.
  • Serve as a point of contact for referral sources and agencies to ensure that effective communication and coordination.
  • Timely and thorough completion of all trainings required by the agency, applicable state regulations and accrediting bodies.
  • Maintain competency to provide treatment through ongoing clinical supervision and continuing education and successfully completion of goals in yearly staff evaluation.
  • Complete assessments on each client accepted, IRPs, biopsychosocial assessments, treatment plans.
  • Provide direct services to clients and their families (i.e., symptom management; assistance with structuring and performing basic daily living skills including money management; development and support of social and recreational activities; consumer psycho education and support family and significant others education and support; client advocacy; crisis intervention; securing and maintaining community integration including addressing housing needs and financial resources; and educates and supports community regarding needs of person with mental illness).
  • Record, report, and monitor service delivery as per program contract requirements.
