Job DescriptionPosition Summary:The Registered Nurse (RN) is a professional nurse specially trained in the Hospice concept to provide for the personal, physical, emotional and social needs of the patient/family unit. He/she works with the Interdisciplinary Group (IDG) to facilitate patient care activities; provide advice, support and assistance to the team members as needed and is responsible for ongoing patient/family evaluation and assessment. The RN is an active participant in the Hospice program including community education.
QualificationsA minimum of one year of clinical nursing experience preferred. Experience must be in Hospice or in a medical/surgical setting, a hospital, oncology or geriatric facility. A current RN license in the State of Florida or Alabama as applicable depending on assignment. Reliable transportation, valid state of Florida or Alabama driver’s license, as applicable depending on assignment, and current automobile liability insurance and insurability under the company liability insurance are required. Must possess current CPR certification.
Job RelationsReports to: Clinical Manager or Director, Branch Operations
Workers supervised: Assigned Hospice Aides, LPNs, Volunteers and Patient/Family units.
Main Duties And ResponsibilitiesCompetently performs all basic nursing skills.
Conducts initial physical assessment of patient in the home.
As a member of the Interdisciplinary Group, is responsible for determining and documenting the level of care status of patients, and in particular, inpatient level of care, to ensure the patient is appropriate and meets the criteria for short-term care needed to manage pain and symptoms in an inpatient facility. This responsibility includes appropriate management and documentation with the patient’s change in status either to short-term inpatient care or removed from that status, and all monitoring of the care during the inpatient period. This same requirement applies to management and documentation of patients who are placed on a Continuous Care level of care. Strict compliance with all rules and regulations, including Covenant Hospice’s policies and procedures, is required. Assists in creating and maintaining a climate that promotes growth and harmonious relationships within the Hospice staff.
Special Demands/Physical Requirements:This position frequently requires medium/heavy lifting. Normally such lifting will involve but not be limited to, assisting with transfer/repositioning of the patient either in the home or nursing facility. The position is also mentally demanding because of the continual need of assessing and evaluating clinical aspects of the patient's care. This position requires considerable time traveling in a motor vehicle in connection with performing essential functions of position.

- Develops and maintains a written Hospice Care Plan for each patient and updates twice monthly or more often if needed
- Makes home visits for physical and emotional assessments of patient/family units.
- Maintains complete, accurate and up-to-date medical records and other records pertaining to the provision of care. Documents all contacts and has documentation in office per policy.
- Communicates on a regular basis to other team members the current status of the patient/family unit.
- Collaborates with IDG members to ensure the best care for the patient/family according to the Hospice concept.
- Develops Hospice Aide plan of care based on patient’s personal care needs.
- Makes Supervisory visits to all patients being cared for by Hospice Aide.
- Requests volunteers as needed in writing.
- Contacts all volunteers assigned to Nurse's patients to:
- Share information before volunteer sees patient
- Update volunteer
- Notify volunteer of IDG meetings
- Assists in volunteer training and in-services.
- Notifies Clinical Manager, Interdisciplinary Groups/Branch/Residence Manager on a daily basis of all changes in status of patient, including hospitalizations and discharges. Writes weekly updates.
- Assists other team members in patient/family care.
