Licensed Practical Nurse - Full Time and PRN

McLean County Nursing Home

Minimum Starting Rate: $25.7728/hour

Shift Differentials for evening, overnight and weekends

Position Summary

The LPN will provide routine care, observe resident’s health, assist physicians and RNs, and communicate with residents and their family members.

Essential Duties and Responsibilities

  • Responsible for nursing care of assigned residents in accordance with nursing facility and nursing service policies and procedures
  • Recognize changes in resident’s condition and notify physician; communicate with family members as needed
  • Supervise resident care personnel
  • Prepare and administer medications and treatment
  • Chart medications and treatments according to procedure
  • Participate in staff meetings
  • Administer parenteral, intramuscular, and sub-cutaneous injections and IV therapy
  • Ensure comprehensive care plan is followed
  • Receive and place calls to physicians; accurately transcribe physician’s orders
  • Admit, discharge, and transfer patients from their assigned unit
  • Complete assigned monthly summaries
  • Other duties as assigned by the Director of Nursing or Nursing Home Administrator

Knowledge, Skills, and Abilities

  • Possess skills for continuous handling, fingering, and feeling required for palpation, operating and maintaining equipment, and providing medical treatments for abnormal conditions, ie: wound care, resident care and medication administration
  • Possess skills to continuously comprehend and communicate with residents and co-workers, to follow written plans of care, and to respond to emergency alarms
  • Ability to respond to and handle emergent situations
  • Ability to concentrate on moderate and/or fine detail with frequent interruptions necessary to perform a variety of tasks
  • Ability to remember multiple messages, information and task assignments given to self and others
  • Ability to work well on a team

Required Qualifications

  • Current, active license as Licensed Practical Nurse in Illinois
  • Graduate of an accredited school of nursing
  • On the job training as an integral part of the licensed training program

Preferred Qualifications

  • 1 year of experience in geriatric care in a nursing home or in other related health care facilities

Physical Requirements

  • Occasionally transfers up to 75 pounds with assist while lifting and positioning and transferring residents
  • Pushes/pulls medication carts requiring force of 10-30 pounds
  • Pushes/pulls residents weighing 75-300 pounds in wheelchairs
  • Reposition residents weighing 75-300 pounds in bed for treatments and nursing procedures as needed
  • Stands and walks up to 75% of shift to provide resident assessments
  • Stoops, bends, and squats as required to reach lower drawers on medication carts, adjust residents’ items, etc.
  • Able to wear gloves and other PPE
  • Identifies residents, call lights, and unusual occurrences on unit at at distance of 50 feet
