Summary:
Facilitates Hospice and Palliative Care referrals from all sources and obtains reviews & coordinates the necessary information to complete the clinical assessment of the patient for the correct program & correct LOC within that program. The position assumes dual accountability for Palliative Care and Hospice referrals.

*Associates Degree or Diploma in Nursing minimum
*New Mexico RN License required.
*Minimum one year of acute care experience required and hospice experience strongly preferred. *Basic typing competency.
*Current driver's license and auto insurance.
