Full-time (36hrs), Midnight Shift
GENERAL SUMMARY:
Under the general guidance of the medical director, the Nurse Practitioner/Physician Assistant will evaluate, diagnose, and treat patients with diseases and adverse health conditions following the written guidelines approved by the department.
Rapid Response Team - Hospital-wide 24/7 coverage of all RRT calls, including ancillary areas. Main responsibility is the efficient assessment and stabilization of patients by ordering and interpreting variety of diagnostic tests, performing therapeutic interventions, and transferring patient to appropriate level of care. All of this done in collaboration with attending and/or consulting physicians, respiratory therapists, nurses, and ICU residents when necessary.
Pulmonary Embolism Response Team - Acute risk stratification for all PE patients in ER or inpatient units for potential interventions. Responsibilities include performing acute assessment of patient then initiating conference call with PERT attending physician, cardiologist, interventional radiologist, and others to determine appropriate therapeutic intervention.
Acute Stroke Team - Responding to all code strokes in ER and inpatient units. Responsibilities include evaluating patient and performing time sensitive diagnostic and therapeutic interventions in collaboration with on-call neurologist, ER physicians, and neuroradiology interventionists.
Transition of Care Notes - Assisting with admitting patients from the ER to the floor by performing transition of care notes, reconciliation of medications, and ordering appropriate diagnostic and therapeutic tests/interventions.
Management of ER Hold Patients - Management of critical lab results, decompensating patients, and responding to all nursing concerns/requests
