Corecare Systems Inc is Hiring a Patient Navigator Near Philadelphia, PA
The Patient Navigator is the primary liaison between the patient and the program to help patients adjust to institutional routines and engage in treatment to prevent early departure against medical advice. The Patient Navigator guides patients through the healthcare process by taking time to understand the patient’s goals and needs. The Patient Navigator ensures that the patient is connected to prevention and treatment services. Help patients understand the importance of completing treatment and successful discharge to prevent relapse/readmission.
Available Shifts: 10 am - 6:30 pm; 12 pm - 8:30 pm
Requirements
- Associate degree in a healthcare-related field preferred.
- High School Diploma or equivalent required.
- Bi-lingual (English/Spanish) preferred.
- Driver’s license and good driving history
- 1-3 years of relevant experience in a healthcare environment; experience in a
- Rehab and/or Psychiatric setting preferred.
- Computer literacy is a must.
- Compassionate attitude and professional demeanor.
- Willingness and ability to support a diverse population of patients.
- Strong organizational skills.
- Strong verbal and written communication skills.
- Ability to multi-task.
