COPC Brand is Hiring a Licensed Practical Nurse- Care Coordinator Near Westerville, OH

Central Ohio Primary Care Physicians is seeking a Care Coordinator LPN to join it's Case Management team. The Care Coordinator, LPN cares for high risk, chronically and acutely ill patients by collaborating with Care Coordination team to monitor care and treatment of patients. This position manages a caseload of roughly 40-60 cases per month and includes home and telemedicine visits. Responsibilities include regular communication to patient and clinical staff to achieve highest quality of patient care and treatment outcomes. The Care Manager, LPN assists in the evaluation of resources and in developing/revising plan of care. This is a full time position working 8a-5p Monday-Friday

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Participate in care of high risk, chronically or acutely ill patients.
  • Provide ongoing feedback to patient primary care team either through regular communication in the patient health record or through visits in the primary care offices.
  • Perform complete assessment of patient's current health status, including barriers to achieving optimal health, and available resources.
  • Assist with the edit, update, and renewal of care plans by providing patient information and clinical data to RN Care Coordinator. Collaborate with care teams and community partners to facilitate continuity of care.
  • Manage a case load of acute and chronic care patients as well as patients who have acute needs under the direction of the RN Case Coordinator.
  • Communicate with patients via phone calls or during scheduled home visits in a timely manner.
  • Assist in Transitional Care Management on as needed basis. Facilitate referrals to other disciplines both internally and externally to improve patient outcomes.
  • Perform other duties as assigned.

QUALIFICATIONS:

A. Experience, Education, Licensures & Certifications 

  • Required: Licensed Practical Nurse in state of Ohio 
  • Preferred: A minimum of three years prior direct patient care experience

B. Knowledge, Skills & Abilities

  • Must be able to perform any clinical or clerical duty as assigned skillfully
  • Must demonstrate the ability to handle stressful situations appropriately
  • Must be able to work flexible hours as needed
  • Must demonstrate ability to manage case load and document progress in a timely way as outlined by the care program assigned to each patient
  • Must maintain patient confidentiality
  • Must demonstrate a proficient driving record and up to date auto insurance as travel to visit patients is necessary
  • Knowledge of chronic and acute disease states in adults
  • Knowledge of common medications used in a primary care setting including indications, dosages, and side effects 
  • Knowledge of trends in healthcare, managed care, Medicaid, case management, medical management ,and quality improvement
  • Proficient in computer software and usage including but not limited to Microsoft Outlook and Microsoft Excel
  • Strong analytical, organizational, and time management skills
  • Ability to work independently and within a team environment with minimum supervision
  • Ability to demonstrate work toward the progress of patient-centered goals
  • Ability to develop and maintain rapport among health care professionals within individual COPC practices
  • Ability to adapt to changing environment
  • Ability to work as part of the team
