Congregational Home is seeking to hire a Full-time 1st shift Registered Nurse Care Manager!

$5,000 SIGN-ON BONUS

Job Responsibilities:

• Plans and coordinates care in a given area.
• Supervises the care given by all nursing staff on all shifts.
• Prepares and delegates assignments to all team members.
• Admits and discharges residents from unit.
• Performs assessments and evaluations of residents upon admission and as needed thereafter. This includes the initial MDS.
• Develops initial care plan, updates care plan, or assures that care plan is updated quarterly and as needed.
• Monitor care plan compliance.
• Coordinates Multidisciplinary Plan of Care Conference.
• Communicates with physicians, family/responsible party, nursing staff, and other departments as needed to assure quality care.
• Evaluates Resident Care through QA rounds on a regular basis, incorporating resident appearance, resident rights, infection control, unit appearance and safety guidelines.
• Assures timely correction of any noted QA problems.
• Documents as needed, and assures documentation is completed in a timely manner including but not limited to: incident reports, admission/readmission, discharges, changes in resident condition, narrative notes, summaries, plan of care charting, MARS, treatment record and MDS.
• Reports significant change in condition, incidents, noncompliance of resident care plan, and quality assurance concerns to Director of Nursing.
• Performs all staff Registered Nurse duties on an as needed basis including but not limited to: receiving and transcribing physician orders, assisting with physician visits, administration of medications, performing treatments, direct nursing care and restorative nursing.
• Evaluates staff members performance in a timely manner and provides necessary documentation including performance appraisals and disciplinary actions.
• Participates in education programs, orientation and general in services as needed.
• Participation as an active member of nursing department through committee participation, QA activities and other duties as assigned.

Qualifications:

• Graduate of an accredited School of Nursing.
• Current license in Wisconsin
• Two years’ experience as staff R.N. Manager in long-term care
