Job Overview:
We are seeking a skilled and experienced MDS Coordinator to join our team. As an MDS Coordinator, you will be responsible for coordinating and overseeing the Minimum Data Set (MDS) process in our facility. This is a vital role that ensures accurate and timely completion of resident assessments, which directly impacts the quality of care provided to our residents.
Responsibilities:
- Coordinate and oversee the MDS process, including the completion of assessments, care plans, and documentation
- Ensure compliance with all federal, state, and local regulations related to MDS assessments
- Collaborate with interdisciplinary teams to gather necessary information for accurate assessment completion
- Conduct comprehensive assessments of residents' physical, mental, and psychosocial status
- Monitor and evaluate resident care plans to ensure they are individualized and meet the needs of each resident
- Communicate with healthcare providers to obtain necessary medical information for assessment completion
- Participate in quality improvement initiatives to enhance resident care outcomes
- Stay updated on industry trends and changes in regulations related to MDS assessments
Qualifications:
- Previous experience as an MDS Coordinator or similar role in a healthcare setting
- Knowledge of memory care, acute care, senior care, pediatrics, and medical office practices
- Familiarity with eClinicalWorks or similar electronic medical record systems
- Strong understanding of MDS assessment processes and regulations
- Excellent attention to detail and ability to work with complex documentation
- Ability to effectively communicate and collaborate with interdisciplinary teams
- Knowledge of acute pain management and ventilator care is a plus
- Experience with dermal fillers is a plus
