SUMMARY:

To assess, plan, coordinate, implement and evaluate Branch operations and patient care management in coordination with the interdisciplinary care team. Patient care to be provided to home care patients by a team consisting of Clinical Supervisors, RN Case Managers, home health aides and other health care delivery professionals in accordance with patient needs, government regulations and company policies to maintain excellent care for the patients. Addresses and coordinates clinical and administrative functions.

ESSENTIAL JOB FUNCTIONS

  • Work with the management staff, Director of Nursing, and Area Director of Operations / Chief Operating Officer (COO) in planning all aspects of care delivery, including establishing
  • priorities and job assignments.
  • Continuously monitor the delivery of nursing care, support services and employee morale to ensure patient needs are being addressed in a proactive manner.
  • Assist in the coordination of quarterly Performance Improvement and Clinical Record Review meetings to ensure service delivery is continuously enhanced.

  • Attends committee meetings and coordinate activities according to policy and ensure recommendations are incorporated into policies and procedures and practice guidelines.
  • Performs periodic audits of the agency’s medical records, personnel files, committee reports, and in service/staff meeting manuals to assure agency performance meets or exceeds state and federal regulations governing home health care.
  • Coordinate on-site evaluations for field employees within ninety (90) days of hire and then annually. Coordinate on-site field supervision, as needed, to pro-actively identify problem areas, verify adherence to agency policy and procedure and ensure that care/service is appropriately provided.
  • Ensure staff and other support resources are appropriately utilized and a high level of interdepartmental teamwork is maintained.
  • Coordinate annual agency evaluations and statistically report to the Governing Body.
  • Assists in interpreting federal and state governmental regulations for all personnel and for agency policy and procedures. 
  • Carry out, coordinate, or manage administrative functions or programs related to clinical service delivery, which may include; coordination of twenty-four(24) hour nursing coverage, seven(7) days a week.
  • Facilitate the review of documentation for compliance with Medicare/Medicaid guidelines, necessity of services and compliance with physician’s orders.
  • Coordinate and conduct and maintain accurate records of all clinical in-house meetings to include Patient Case Conferences, Clinical Staff Meetings and In-service Education Programs.
  • Provides input to assist the PCC in the evaluation of Company personnel performance.
  • Ensure current and future staffing through development of recruiting sources; through appropriate selection, orientation, training and continuous staff education.
  • Adhere to all of the Company’s policies and procedures.
  • Performs other duties as assigned.

QUALIFICATIONS

  • Must be a licensed Registered Nurse, Bachelor’s degree preferred
  • Must have at least two (2) years management experience with a minimum of one (1) year home health or hospice and supervisory experience
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
  • Must be able to relate positively and favorably to the patients, families and outside agencies and to work cooperatively with other employees
  • Must be capable of performing the job functions of this position with or without accommodations
