Field Based Case Manager

Concourse House, HDFC is seeking case -managers to work in the field with formerly homeless families who are now living independently, but will benefit from support services. The case manager will provide on-going social services to the client and family as needed.

Responsibilities:

* Assess client’s needs and develops initial and on-going clinical plan for care, follow up and update accordingly

* Assist with the yearly apartment recertifications

* Maintain detailed, accurate and timely progress notes and other documentation

* Assistance applying for and securing public benefits and entitlements

* Provide linkage to resources and make referrals to community agencies

* Advocate and provide assistance with all social, educational and health needs

* Work with team members and provide outreach for and engage resistant/hard to reach clients

* Assist with maintaining a healthy household by developing a plan to keep the home in good standing both physically and financially

* Knowledge of current housing programs and vouchers, assist with arrears and relocation when necessary

* Escort clients to their appointments as needed

* Outreach and Training as well as Network and liaise with outside agencies and make referrals

* Attends staff meetings, trainings and supervisory sessions as required

* Perform and manages effective case management

Bachelor’s degree in Human Services or related field preferred. MSW a plus. Experience providing direct service to clients with mental and social issue, chronic poverty, developmental or substance abuse disorders, or chronic medical conditions, effective Communication skills required and bi-lingual in Spanish a plus. Pay: $53,000.00 - $55,000.00 per year
