The Registered Nurse Supervisor (RN) oversees the management and coordination of the day to day operations involving the Home and Community. Provides appropriate end of life nursing care to all patients and families based on individual plan of care, and in accordance with established regulations and administrative policies.

QUALIFICATIONS FOR THE REGISTERED NURSE (RN) SUPERVISOR POSITION:
- Registered Nurse (RN) with a current valid Pennsylvania license.
- Two (2) years of progressive experience recommended.
- Interpersonal, communication and organizational skills.
- Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state
RESPONSIBILITIES FOR THE REGISTERED NURSE (RN) SUPERVISOR POSITION:
- Reviews clinical documentation for compliance and completeness in a timely manner.
- Reviews Plan of Care (485) for accuracy and completeness.
- Review HIS metrics
- Oversees and offers supervision to members of their team to help assure that quality of care is provided.
- Holds regular meetings to provide education to team members and facilitate interdisciplinary communication.
- Acts as liaison between patient/family, physician and all other healthcare providers.
- Provides in-home nursing visits on an as-needed basis.
- Participates in administrative on-call rotation.
- Collaborates with the patients and the clinical staff when needed when abnormal reading or trending is identified.
- All other duties as assigned/requested.
