Concordia-IRMC VNA is seeking a Registered Nurse Liaison/Marketing for our Indiana Office.
POSITION SUMMARY
The Transitional Nurse Liaison is responsible to coordinate all offered services for home healthcare/hospice as applicable. This may include completing part or all of the admission process for patients. Partners with the sales team in developing and maintaining referral source relationships. Their main responsibility is discharge planning to the agency from facilities.
QUALIFICATIONS
  • Current Pennsylvania Licensed Registered Nurse.
  • Bachelor’s degree in nursing recommended.
  • A minimum of three (3) years nursing experience required. Supervisory and acute care discharge planning experience preferred.
  • Case manager certification and/or specialty certification preferred.
  • Maintains current PA Driver’s License and auto insurance.
  • Strong clinical assessment skills.
  • Skills in typing and organizational abilities.
  • Preferably has home healthcare experience.
  • Able to work independently, demonstrating good judgment.
  • Must be accurate and detail oriented.
  • Practices good customer service skills in all working relationships.
  • Desire and ability to learn.
  • Strong interpersonal skills, respectful, and courteous nature.
  • Absence of criminal background.
DUTIES AND RESPONSIBILITIES
  • Provides on-site consultative services to area hospitals and facilities regarding adult and geriatric patients referred for homecare/hospice services, and referrals for post acute rehab and skilled nursing facility (SNF) care.
  • Provides comprehensive adult and geriatric assessments to assure development of thorough homecare plans or discharge plans to Concordia-IRMC VNA. Utilizes strong clinical assessments skills to facilitate the development of plans for patients of all ages and medical needs.
  • Coordinates/documents adult and geriatric assessments prior to discharge to Concordia-IRMC VNA.
  • Coordinates homecare/hospice needs to assure quality patient care and delivery of services: helps promote information transfer between levels of care.
  • Attends interdisciplinary clinical meetings at institution as requested.
  • Demonstrates timely and accurate computer documentation/written documentation of patient information as well as daily activities to Concordia-IRMC VNA branch staff via the contact relationship management software (CRM).
  • Provides timely response to all intake inquiries at assigned institution; works closely with Intake and Clinical Management.
  • Identifies, develops and builds upon referral sources within assigned institution; develops relationships with other referral sources in market area.
  • Assists in the identification of new services that are needed or should be expanded based on experience/input at institution.
  • Educates markets and promotes to patients, families and referral sources the Concordia Lutheran Ministries continuum of care.
  • Provides education, supervision and counseling to the adult and geriatric patient/family regarding expectations of homecare/hospice plans.
  • Performs work under the supervision of the Agency Management.
  • Answers the phone, takes inquiries or messages using professional telephone technique.
  • Attends required and appropriate office meetings.
  • Maintains a good working relationship with all staff.
  • Prepares and places ads for recruitment of clients.
