Description
Comunilife Inc. is looking to hire an experiencedSocial Worker to join itsBronx Congregate program, EL Rio II. The Social Worker will conduct psychiatric and other assessments, as well as develop and implement person-centered treatment plans, and supportive services. Services are delivered through evidence-based client treatment practices. Essential Duties:
Assess clients substance abuse, health, mental health, and other care needs; recommend internal and external referrals;
Provide individual and group counseling;
Prepare psycho-socials;
Facilitate psycho-education groups in relapse prevention, health literacy, and navigating mental health and substance abuse systems, etc. for residents their family and/or support networks;
Provide Comprehensive Case Management services;
Assist clients in improving and maintaining self-care, interpersonal, and vocational skills
Assist clients in achieving housing stability
Manage client documentation, provide program narrative and/or statistical reports as needed;
Participate in ongoing QI and program evaluation activities
Qualifications
Education and Experience Requirement(s):
Masters degree in Social Work (MSW) required; New York State Licensed Masters in Social Work preferred;
Two (2) years experience with homeless populations;
Two (2) years experience with persons with chronic health and/or mental health conditions and/or substance abuse disorders;
Bilingual/Spanish skills a plus

Comunilifes affordable and supportive housing programs address the need for safe, affordable, service enriched housing for a low-income population that has difficulty maintaining stable housing. A majority of our housing clients have a history of homelessness and/or long-term hospitalizations. Many are living with mental illness, HIV/AIDS and other chronic illnesses. Comunilife has 1806 apartments/units in 10 supportive residences, 3 low-income apartment buildings and 981 scatter site apartments in the Bronx, Brooklyn, Queens and Manhattan. Our Medical Respite program provides short-term supportive housing and case management services to multiply-challenged patients leaving hospital care without a viable home in which to recuperate. Comunilife is a leader in the field of Latina adolescent suicide prevention. Our Life is Precious Program, which is celebrating its 10th anniversary in 2018, provides non-clinical services to girls ages 12 to 17 in the Bronx (2008), Brooklyn (2009) and Queens (2015).Activities include academic support, creative arts therapies, music lessons, wellness activities and concrete family services, are designed to reduce the risk factors associated with Latina adolescent suicide.
