Comunilife is Hiring a Clinical Supervisor-Scatter Site (OMH) Near Bronx, NY

 
 

Comunilife is looking for an experienced Social Worker Supervisor to join their scatter site team in the Bronx. The Clinical Supervisor is responsible for the supervision and training of the case management staff of the OMH Scatter Site Housing Program. Responsible for the coordination of the delivery of social support services to all clients. The Clinical Supervisor also assists with the intake process of new clients. Major responsibilities include the administration of the program, monitoring of program services, building relationships with other agencies, and the supervision and evaluation of social service staff. This position is eligible for a generous benefit package including health insurance, dental, vision, dependent care, paid time off including 4 weeks of vacation starting in the first year, 401(k), tuition assistance program and more. The salary range for this position is up to $65,000 a year.

ESSENTIAL FUNCTIONS AND KEY ACCOUNTABILITIES

  • Assign cases to case managers and schedules work according to priority;
  • Maintain necessary records and supervision notes; prepare regular and special reports as required;
  • Oversee quality assurance of case management services to ensure compliance with Comunilife, and government policies and regulations;
  • Performs audits of client charts to ensure comprehensive services planning and accurate charting of client progress;
  • Ensures that the home visit policy and procedures are enforced, tracked and documented.
  • Provide training to new and current case managers;
  • Speak to dissatisfied clients and handle difficult situations;
  • Develop and maintain cooperative relationships with ancillary agencies;
  • Prepare and submit all required program statistics and reports accurately and in a timely fashion;
  • Under the supervision of the Program Director, develop comprehensive group programming, in compliance with contract and agency requirements;
  • Coordinate and supervise efforts with case management and maintenance staff to ensure a minimum monthly occupancy rate of 95% is maintained;
  • Coordinate and supervise efforts to place new OMH clients in vacated units within a maximum of 30 days from referral;
  • Ensure that the initial and medical assessments are conducted, and that comprehensive service plans are developed that identify mutually agreed upon long and short-term goals;
  • Review and provide supervision in the development of case management service plans and ensure that reassessments are conducted on a quarterly basis (at minimum) to ensure that services are appropriate for the client’s current needs;
  • Attend allstaff and community meetings;
  • Assume a caseload in the absence of a case manager.
  • Conduct apartment inspections periodically to ensure client safety and appropriate living environment. 
  • Coordinate and oversee the discharge plan process of hospitalized clients. 
  • Perform other duties as assigned and/or needed by PD.

Education and Experience Requirement(s)Minimum Bachelors Degree in Social Work, Human Services, Education with at least two (2) years to five (5) years supervisory experience, overseeing a case management team. Individual, family and group counseling experience required, particularly with HIV/AIDS affected populations and/or substance abuse individuals. Preference given to persons with experience in Scattered Sites Programs, bi-lingual, bi-cultural in Spanish.

  • Extensive knowledge of principles and practices of OMH case management supervision.

  • Knowledge of OMH guidelines and regulations.

  • Responsible, organized, effective, and efficient. Ability to perform multiple assignments simultaneously.

  • Skill to communicate effectively in writing, vocally, and to listen actively.

  • Community minded, patient, creative, flexible, compassionate, and culturally sensitive to persons who are diagnosed with severe and persistent mental illness.

  • Ability to deal tactfully and diplomatically with other Comunilife employees, Federal, State, and City officials, professional and technical groups, clients, and the general public.

Certifications & Licenses Preferred

HIV/AIDS pre-test and post-test certificates. Licensed Masters in Social Work (LMSW) or New York State Licensed Certified Social Worker (LCSW).
