Compass Care is seeking an energetic, flexible, and entrepreneurial Licensed Practical Nurse (LPN) to join our Clinical Team! This team is responsible for identifying and coordinating needed services as dictated by the medical treatment plan, nursing assessment by an RN and the client environment, with the result being exceptional care for each client. What We Seek In Our RNs/LPNs:

• Current CT, NY, and/or NJ nursing license
• Minimum of 1 year of experience as an LPN.
• Excellent written and oral communication skills
• Ability to support caregivers in the field.
• Ability to work cohesively under the supervision of an RN as needed.
• Experience with older adults and in-home care.
• Institutional experience and experience with dementia are a plus.
• Prolonged periods sitting at a desk and working on a computer.
• Must be able to travel throughout Compass Care service areas. Travel may include, but is not limited to, driving, public transportation, walking, stairclimbing.
• Must be able to lift up to 50lbs at times.

Responsibilities:

• Clinical field support and training of caregivers to ensure a high level of clinical awareness amongst our caregivers.
• Regular visits and monitoring of clients as requested by an RN.
• Professional manner interfacing and coordinating with clients, families, and physicians as needed.
• Working with the entire clinical team to develop new quality assurance processes.
