Education required: Bachelor's level or higher in a Human Service field (ie. psychology, sociology, philosophy, family and consumer science, social work, etc.) -OR- Bachelor's degree or higher with a minimum of 16 credit hours in the Human Service field on official transcript.
Responsibilities: Performing case management (or support coordination) duties for clients with developmental disabilities and their families. This includes, but is not limited to: monthly phone calls, quarterly in-home visits, annual plan of care meetings, follow-up meetings, and occasional pop-up visits to homes and day habilitation / pre-vocational facilities.
Applicants should have:
· Bachelor's degree or higher from a 4-year accredited college
· Clean background check
· Clean driving record
· Own transportation and current insurance
· Excellent communication skills, both verbal and written
