Community Partners is a non-profit organization that provides critical supports and services for individuals and families living with developmental disabilities and/or behavioral health challenges. An in home personal care provider works with children and adolescents with intellectual disabilities. The provider will work 1:1 with the individual on goals established as part of the individuals In Home Supports plan. Goals may focus on but are not limited to increasing independence with activities of daily living, increasing social/communication skills and gaining life skills. The ideal candidate will have experience working with individuals with disabilities, in particular autism, and is comfortable identifying support needs. The ideal candidate will be someone who is energetic, observant, a great communicator and able to make community connections on behalf of the child they support. We currently have a number of individuals in need of personal care support across Strafford County, varying in age, levels of support needed and number of hours needed per week. Transportation will be a requirement of this position. Candidate must possess a valid driver’s license, a clean driving record and proof of insurance. All candidates must pass a background check which includes, but is not limited to, criminal and driving records to be considered for the job. Community Partners, Strafford County’s premier behavioral health & developmental services provider, links people to the long-term supports and services they need to a live a full life in this community. Experience in any of the following areas will be considered: LNA,CNA, DSP, Direct Support Professional, patient care, client care, aide, paraprofessional, youth counselor, residential support, childcare, disability, brain injury, brain disorders, mental health, nursing, medical provider, treatment provider, elderly , hospice, caretaker, homemaker, home health, home provider, PCA, companion care, home health aide, nanny educator, teacher, resident, behavior support, or para educator.
