Nurse Practitioner, Telehealth/PACE

Community PACE at Home is seeking a Nurse Practitioner for Telehealth Visits / Clinic position. This individual will be responsible for providing direct and indirect care to our PACE participants in the Newaygo area. Our NPs work under the direct supervision of the Medical Director/PCP as described in the Nurse Practitioner Collaborative Practice Agreement. This position is being offered as hybrid and is full-time salary.

To be successful in this position flexibility is necessary. We are looking for a Nurse Practitioner that will be based out of our Home Health Agency in Muskegon and providing services at the Community PACE at Home Clinic in Newaygo. This is perfect for an individual that is looking for a varied routine and a flexible schedule. There will be work in the clinic, telehealth visits, and home visits from time to time as well. There will be no one day like another.

One of the benefits of PACE is that our patients can receive care that is otherwise “not typical.” With that, it will be expected that the scope of your job may not be “typical” at times. Whether that includes delivering items that participants need to their homes, providing care, staying outside of “typical” hours with a participant to ensure participant safety, performing in home visits/checks as determined by IDT and any other “outside the box” interventions while remaining within your scope, that may come up to provide the best possible outcome for our participants. Requirements

• Master's Degree in Clinical Nursing from an accredited Nurse Practitioner Program.
• RN License in Michigan and a Family Certification from an accredited Nurse Practitioner Program.
• 1-2 years’ experience as a practicing Nurse Practitioner preferred
• 1 year of experience with a frail or elderly population.
• Cleared medically of communicable disease.
• Ability to learn, research, problem solve, and think analytically.
• Able to work within the Interdisciplinary Team (IDT) setting.
• Knowledge of physical, mental, and social needs of frail older adults.
• Knowledge of Medicare and Medicaid.
• On call rotates with other providers, currently a two week on / two week off schedule but that will change with additional providers added.
