Job Title : SUPPORTS COORDINATOR – FAMILY SERVICES/CHILDREN

Job Summary :

The Supports Coordinator assigned to the Family Services team coordinates supports and services for children/adults living with their legal/biological/adoptive-foster families. The Supports Coordinator serves as the primary advocate and broker of services within the system for the people he/she/they support. He/she/they work within the mental health system and the community to ensure the people supported have maximum access to and use of the resources available. The Supports Coordinator focuses efforts on connecting the children/adults supported to networks of family, friends, and meaningful relationships, and develops strategies to coordinate the individual’s personal resources with entitlements and special programs.

The Family Service Supports Coordinator provides these services while adhering to the principles Person Centered Planning, Family Centered Planning, the Michigan Mental Health Code, Detroit Wayne Integrated Health Network (DWIHN) and Michigan Department of Health and Human Services (MDHHS) requirements, Self-Directed Services Technical Advisory, CLS Guiding Principles, Code of Ethics, and professional code of ethics.

Reporting Relationship : Reports to the Family Services Supports Coordination Manager

Duties and Responsibilities :

The Supports Coordinator assigned to the Family Services team is responsible for the following:

  • Meets with the individual in accordance with the amount, scope, and frequency noted in their Individual Plan of Services (IPOS) and additionally, as the individual’s needs and choices dictate.
  • Assists the families, along with the children and adults, in building circles of advocates, friends, family involvement, and identification of life choices as well as strategies to fully achieve their goals.
  • Works collaboratively and cooperatively with CLS employees both within and outside of the Family Services Department, the overall Supports Coordination Department as well as outside entities to ensure services are met for individuals supported.
  • Completes all documentation with a high degree of quality and in a timely manner consistent with CLS agency requirements and other external funding sources.
  • Assists the individual/family in accessing entitlements such as Social Security, Family Support Subsidy, Children’s Specialized HealthCare Services, Adoption Subsidy, Children’s Waiver (for those Supports Coordinators who work with Children on the Waiver), Autism Benefit Waiver, Wraparound Services and Home Help.
  • Links, coordinates, and provides monitoring of services as identified in the IPOS and as communicated by the person, their family and circle members .
  • Assists with pro-active planning on behalf of the individual related to potential crises situations and provides crisis resolution support as needed.
  • Assists the individual and family to coordinate and secure services needed to address any health concerns. Coordinates health and safety strategies, including planning related to emergency needs and coordination with Medicaid Health Plans and other health care providers. Coordinates discharge planning as needed for hospitalizations.
  • Assists the individual to identify goals and develop strategies to support the goals both logistically and financially. Assists with goal development, fulfillment, plan implementation, and follow-up per funding requirements.
  • Assists the individual to become an active and contributing member of their community.
  • Assists the individual with the development of self-advocacy skills.
  • Works collaboratively with other service providers and supporters in the individual’s life to develop individualized, creative methods of service delivery specific to the individual’s needs and desires.
  • Assists the individual and family to coordinate and secure transportation as needed.
  • Coordinates on-going supports with school system. Participates in school transition planning related to the individual’s post-graduation aspirations.

Administrative:

The Support Coordinator on the Family Services Department is also responsible for the following:

  • Using agency issued laptop computers and data base programs to complete all documentation.
  • Accessing CLS voicemail, business cell phone, standard mail, and email on a daily basis.
  • Completes required reports and summaries of services for funding sources as needed.
  • Completes a minimum of 5 hours of mandatory Qualified Intellectual Disabilities Professional (QIDP) training each year.
  • Complete Category of Care and Pre-Screen (exclusive to Supports Coordinators who work with Children's Waiver)
  • Participate in Individual Education Program (IEP) meetings as requested by the parent for purpose of coordination of care.
  • Ensure monthly Continuity of Care with the autism Providers for those individuals receiving Applied Behavior Analysis (ABA) services.
  • Supports Coordinator will make a referral to Wraparound Program Services for a child/family who are in need of more intensive services.
  • Assists individuals, families, and other support providers in locating and obtaining necessary community resources and utilizing natural supports as available, prior to accessing public funding. Demonstrates a commitment to the prudent use of public funds.
  • Participates in relevant workshops, seminars, conferences, trainings, and in-services as approved by management and required by agency.
  • Completes timely Individual Plan of service, (IPOS), Addendums, Periodic Reviews, Integrated Biopsychosocial Assessments, Progress Notes, and other documentation necessary to memorialize service delivery and identified courses of action per agency and funder’s requirements.
  • Meets performance requirements related to job duties and service delivery. Completes required written reports and documentation in a quality and timely manner consistent with requirements of CLS agency and external funding sources.
  • If supporting children who are enrolled in the Children’s Waiver Program, follows all program and documentation requirements, and submits information and documentation to Waiver Support Application (WSA) within required time frames.

Qualifications and Requirements :

Education/Experience:

Minimum Bachelor’s degree in Human Services field with minimum one-year experience, or combination of field practicum and experience, working with persons with intellectual/developmental disabilities (I/DD). Experience working with children and families within the foster care and/or adoption system preferred but not required. Must minimally meet education, and experience requirements for provisional credentialing as a Qualified Intellectual Disabilities Professional (QIDP); full QIDP credentials preferred.

Training:

  • Must achieve and maintain Qualified Intellectual Disabilities Professional (QIDP) credential, including minimum of 5 hours of QIDP-designated training each year.
  • Must complete other mandatory, specialized training as required (e.g., Children’s Waiver Category of Care)

Travel Requirement:

Extensive scheduled and non-scheduled travel is required in executing essential duties and responsibilities of this position. Employees must provide their own transportation. Maintenance of a valid driver’s license, an acceptable driving record, and vehicle registration and insurance in compliance with State of Michigan requirements are all continuing conditions of employment. If driving a vehicle other than the one listed on file with the Human Resources Department, employee is responsible for ensuring the vehicle is registered, insured, and meets all other State of Michigan motor vehicle requirements and providing documentation as requested.

CLS policy restricts the use of an alternate driver without the express, prior written authorization of the Human Resources Director. A review of such request will be based on evaluation of HIPAA compliance issues, work restrictions and other relevant factors.

Physical Demands:

  • Physical tasks are minimal as this is essentially a professional field position.
  • Flexibility in scheduling is a must.
  • Ability to be available in the office as well as in the field is necessary in order to complete other essential duties and responsibilities.

  • Criminal background and Michigan Sex Offender Registry checks
  • Protective Service Clearance through the Michigan Department of Human Services
