WHAT WILL I BE DOING?
CHI is looking for a full-time Senior Case Manager for our Middle Country location. You will be responsible for all supervisory aspects of social work services within emergency housing setting. Under the direction of the Program Manager, the Senior Case Manager will work as part of an interdisciplinary team. The Senior Case Manager will supervise the overall running of the Case Managers for their site. The Senior Case Manager will ensure quality housing services are provided effectively and in accordance with guidelines established by CHI, OTDA, Suffolk County Department of Social Services, and other funding sources as appropriate. This is accomplished by performing the following functions:
