The RN Case Manger is responsible for planning and managing an assigned caseload of nursing care and home healthcare patients in and around the Gainesville, FL area. As a member of an assigned care team, the RN Case Manager coordinates resources and performs a wide range of skilled nursing activities for an assigned caseload of adult patients.

Primary Responsibilities:
the RNCM is responsible for ongoing assessments of patient/caregiver needs, performing symptom management, teaching appropriate care related to disease process, and providing end-of-life care/support to patients and caregivers.
Education/Experience:
