Job Responsibilities:

• Providing companionship is the main responsibility.
• Ensuring the member is comfortable, offering drinks or snacks
• G-Tube experience is preferred but willing to train the right candidate (for dinner and meds as needed)
• Transport and partake in Community outings using a wheelchair accessible minivan.
• Light housekeeping: dusting, laundry, dishes, vacuuming etc.
• Additional backup hours may be available and discussed at time of interview with the member.

Ideal candidate:

Must be 18 years of age or older

Must pass a background screening

Must have a valid driver's license

6 months of personal caregiving/CNA experience but willing to train the right candidate.

Maintain a friendly attitude

Be cat friendly

Wage: $19.00 per hour

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self Direct) throughout Wisconsin. Self direction gives program participants the choice over who provides services and how they recieve them, Community Horizons is not the employer on record, members make all the employment decisions with the involvement of a third party fiscal agent responsible for payroll.
