Community Horizons LLC. offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants a choice over who provides services and how to receive them. The members make all of the hiring decisions.

Care staff is needed on the following daily shifts:

830 am – 930 am

1 pm - 2 pm

4 pm - 5 pm

830 pm – 930 pm

Flexible hours, you can choose what hours work best for you!

Pay: $17.00 per hour

During a shift, you should expect to:

• Use a power-assisted sit-to-stand lift.
• Chat about your day with someone excited to listen.
• Warm food in a microwave.
• Provide hands-on assistance for eating.
• Provide personal care.
• Help with medication (pre-package).
• Toss laundry into the machine or fold laundry if time allows.
