Community Horizons is assisting with the recruitment process of a care partner to assist a member of the community with shopping and light housekeeping. Have you explored Self-Directed Services where you will find Less Stress-No Forced shifts-working 1:1 in the individual’s home?
Hours: 4 hours per week (preferred after 1 pm)
Pay: $15 per hour
Tasks include but are not limited to:
· Shopping
· Clean Kitchen
· Clean Bathroom
· Mopping
· Meal Prep
· Taking out garbage
· Laundry
· Medical Appointments
Ideal Candidate:
· Good Sense of humor
· Reliable
· Excellent communication skills
· Reliable transportation
· Valid Driver’s License
· Must be able to pass a background check.

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them.
