This position is within the self-direction program. Under self-directed services you work directly for the individual, which offers more work life balance and autonomy in your work day. This position is hired directly by the member/guardian and not through an agency.
Hours available: Monday – Friday, 4:00pm – 8:00pm. No weekends!
Wage: $15.00/hour
Tasks may include, but are not limited to:
- Assistance with getting up in the morning, including changing clothes and assisting with personal cares
- Hands on support with toileting/cleanup/changing of incontinence products
- Showering/bathing assistance (verbal cues and hands on assist)
- Medication administration and monitoring for adverse side effect
- Light meal prep and helping with feeding via cues and hands on support
- Some light cleaning will be required
Who we’re looking for:
- Someone who is reliable, dependable, and friendly
- An individual comfortable working with those on the Autism spectrum (experience preferred!)
- An acceptable background check
- Great communication skills and ability to take direction

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record; members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
