Position Summary: The Perinatal Services Program Assistant provides interim support as a direct service team member to alleviate program burdens by serving as back up when critical vacancies arise. This role will provide orientation and support for all perinatal patients to accomplish adherence to standard visit schedule and closed loop connection to medical and social resources as needed. This position will be located our Conway Health & Resource Center in SW, DC. Ability to travel to other COH sites as needed is required.

Highlighted Duties and Responsibilities: 

Program Support:

  • Facilitates the objectives of the Maternal & Child Health (MCH) program by maintaining optimal access to supplies and resources needed, completing administrative and operational audits and tasks for programming and team, supporting the maintenance of referral and resource partnerships, and facilitating the meeting and retreat needs of staff. 
  • Provides perinatal services coverage during vacancies in the Perinatal Navigator, Perinatal Care Coordinator, WIC Administrative Assistant, and/or Group Care Coordinator positions by providing appropriate supportive and educational services. 
  • Manages registrations, maintains Zoom links and calendars, helps with distribution of promotional materials, organizes refreshment options (exploring caterers, placing catering orders, coordinating catering arrival and set up) etc. to support group education sessions. 
  • Maintains and oversees MCH program supplies to ensure they are well-stocked, orderly, and appropriate to the needs of the programs. This includes maintaining inventory and ordering supplies, managing the receiving of new items, managing the movement of items to different clinic sites, communicating with vendors and internal staff to coordinate purchases and returns, and securing/safeguarding items to prevent damage, loss, or theft. 
  • Facilitates the receiving of donations via Development-- corresponds with Development regarding donations, facilitates arrival and safeguarding of items, maintains MCH donation wish list based on program needs
  • Provides support to MCH Leadership in managing partnerships (i.e. DC Diaper Bank, DC Health’s Safe Sleep Program) by serving as primary pick up person for material resources, organizing and coordinating volunteers, compiling distribution data, and maintaining inventory and organization of material resources. 
  • Provide administrative support to senior leaders- Reservation of meeting spaces for team meetings and retreats on and offsite, ensuring calendar invites are sent and remain up to date, confirming presence for guest speakers and providing meeting details, coordinating with Facilities for room set up and cleaning, vendor coordination, scheduling ad hoc meetings and interviews, coordinating travel arrangements for employees, and facilitating staff recognitions and celebrations. 

Patient Care:

  • When providing coverage to perinatal in-clinic positions orients all perinatal patients of COH to model of care—individualizing standard visit schedule based on gestational age including Centering, transition to primary and pediatric care. Assesses barriers to adherence to recommended prenatal appointment schedule. 
  • Orients all perinatal patients to COH and external support programs and services that can increase adherence to needed care and improve maternal and child health outcomes (e.g., doulas, Home Visiting, dental care, Emotional Wellness, WIC, lactation education and consultations, car seats, cribettes, parenting groups, Family Success Center, sexual health care coordinators, etc). Collaborates with patient to identify most beneficial programs to make referral. 
  • Provides health education to patients in variety of settings, with an emphasis on increasing understanding of risk factors and warning signs for preterm and low birth weight births, perinatal health, self- management of care, infant health/care, and the health care system.
  • Utilizes CRISP to monitor ER and hospital admissions for all perinatal patients. Provides telephonic outreach to patients to support appropriate utilization of emergency care, timely initiation of OB care, and completion of follow-up care. 
  • Ensures all prenatal and postpartum patients utilize referrals provided through regular reminder calls. Assists with provider appointments, including but not limited to, following up/rescheduling of missed appointments and ensuring access to care by remediating barriers.
  • Co-facilitates, attends, and assists groups (CenteringPregnancy, pediatric group care, Parent Cafes, infant massage) when there are vacancies.
  • When providing coverage in the WIC Administrative Assistant position educates applicants regarding their rights and responsibilities, including the food delivery system, WIC foods, certification schedule, and assists with group education activities as needed
  • Assists as a liaison between the patient and their families, provider, clinical staff and other health care professionals.

Requirements

Minimum Qualifications

  • High school diploma or equivalency required.
  • Experience providing social services, care coordination or health care education required.
  • Ability to prioritize and problem solve independently.
  • Strong written and verbal communication skills.
  • Demonstrated comfort with navigating and entering data into electronic information management systems (databases, spreadsheets, electronic medical records).
  • Experience in community health center setting preferred.
  • Ability to travel between other COH locations and travel between COH sites required.
