Position Summary:The Family Case Manager is accountable directly to the Home Visiting Coordinator. This position provides home visitation services and ensures home visitation activities meet requirements of the DC Health grant and the Healthy Families America evidence based home visiting curriculum. The Family Case Manager is responsible for providing advocacy and support services to the families to promote maternal and child health, reduce toxic family stressors, and promote strong parent/child bonds. Activities will include recruitment, developing Family Support Plans, supporting families in achieving health and stability, coordinating appropriate referrals, and integrating the family with other community resources and agencies. This is a full-time position.
Salary: This position starts at $52k and up based on experience. Indeed provides their own estimated salary calculator and is not affiliated with COH's range.
Highlighted Duties and Responsibilities:
  • Develops Family Support Plans, that are updated at least monthly and signed by clients and support worker,, which incorporate goals for both adults and other children in the family if appropriate. Emphasis on ensuring appropriate health care goals.
  • Meets with families on case load face to face in the home environment to review progress on Family Support Plan, and to follow-up on tasks and services. Frequencies of visits are based on HFA standards.
  • Networks with COH and other community resources for housing, employment, substance abuse treatment, mental health treatment, training, child and family services, etc., and makes referrals as appropriate.
  • Completes required assessments and screenings with families at designated intervals, e.g. Parent Survey, ASQs, PHQ, etc.
  • Meets weekly with the Family Case Management Coordinator for 90 minute reflective supervision sessions. Communicates about client and case management issues in person, via e-mail, or by phone with supervisor as needed outside of regularly scheduled meetings.
  • Attends clinical, departmental, program and care team meetings and other organizational meetings and trainings as appropriate.
  • Maintains appropriate documentation in files, including updated notes, current release of information forms, incident reports, referrals, assessments, etc.
  • Works with outside groups, agencies, or individuals to coordinate family activities in addition to attending events as needed.
  • Completes other duties as assigned by supervisor.
  • Makes referrals for health services to Community of Hope as needed.
Minimum Qualifications:
  • High school diploma required.
  • A.A. or B.A in Human Servicers field (social work, early childhood education, family studies, or psychology) or equivalent professional experience preferred.
  • For Bachelor’s of Social Work degree or Master’s of Social Work degree holders, the appropriate DC license must be obtained prior to start date.
  • Experience providing public health education (ex. reproductive health, smoking cessation, child development, and/or parenting) to at risk populations.
  • Demonstrated cultural competence in communicating with underserved, diverse populations.
  • Ability to conduct home visits. Valid driver’s license and vehicle required, as well as proof of auto insurance required.
